I know Sunderland have had some poorly handled situations off the field lately but I think Blackburn might beat us here.

They tried to sign a lad from Orlando. Completed all the paperwork an hour before the deadline and then forgot to send it to the EFL.

Their owners have had their assets frozen by the Indian government causing cash flow problems for the club, and on top of that their manager has been banned from speaking to the media to prevent him slagging the club off publicly for selling players behind his back and cutting the budget
