The only real fix is stop the clock the second a ball goes out of play and restart it once its back in.
Also not on the ref to do time keeping, this should be done by another person away from the pitch so the managers and players cant get in their ears.
I remember reading a few years ago that a Stoke game had 49 actual minutes of play and thought they would address it. Sadly this season is apparently the worst for in play time ever
