Vladimir Komarov. The Russians built a ridiculously shoddy spacecraft and the capsule he was in was faulty in hundreds of ways - not least of which were the defective parachutes. He came through the atmosphere and hurtled into the ground in the capsule at full speed and was molten on impact.

However he was also fully conscious as he was crying with rage at the Russian space centre who he was in constant contact with.
