Can a person request them?

Your own, yes. Speak to your GP's receptionist about getting access to your full primary care (i.e. GP) record. Secondary care records (i.e. Hospital) may need you to get in touch with the record keeper at the hospital itself.

You can use something called Patient Access to view some of your primary care records online. Again speak to your GP receptionist.
 
I needed to find my immunisation records from when I was a child, sent a email requesting them to child health services and got sent a letter with history in the post in less than a week!
