Can a person request them?

Your own, yes. Speak to your GP's receptionist about getting access to your full primary care (i.e. GP) record. Secondary care records (i.e. Hospital) may need you to get in touch with the record keeper at the hospital itself.

You can use something called Patient Access to view some of your primary care records online. Again speak to your GP receptionist.

You can. Their might be a small charge but it will be nowhere near the cost of getting you what you want.
