I think there’s more to it than just this. If you’ve made your fortune partially by being controversial in the public eye, it’s arguable that it’s then difficult to step away from the public eye and all the corresponding scrutiny that brings. Of course, she can do what she pleases within the constraints of the law, but if you court attention then you can’t cry wolf if the response isn’t what you want or expected.
I think it would be far more controversial and edgy not to do all this stuff to your face, unfortunately now, Madonna looks no different to those B list celebrities and Love Island contestants that you see everywhere..In many ways doing this to your face is just mundane and pedestrian and I would have thought Madonna would want to avoid that
 
I read that Madonna’s response to people (tbf mostly arseholes) commenting that she doesn’t look like Madonna anymore due to the amount of work she’s had done on her face is that they are all ageist misogynists.

Does this mean that anybody who thinks i look like a baldy old twat in my bucket rain hat and anorak is an ageist, body shaming, man-hater?

Or do these things only apply to celebrities, or people who post pictures of themselves looking like twats on the inter web?

I’m curious is the only reason.

Both of you look old but one of you used to be a highly talented beautiful woman who everybody wanted to shag.
 
The word misogynist is now banded about so fuckin out of context, once again showing the complete lack of perspective in modern life.
So what does she call the women who glance at her and think she has gone too far. It is just a word she uses to shut down adverse comments.
