These mistakes are too common though, could there no be some guidance being issued that's causing this considering it's likely to be taking place elsewhere? Why have the families been treat the way they have? Smacks of more than mistakes.

I wouldn't say they're too common otherwise there would be more reports of it happening at other hospitals.
This seems to have been a catalogue of errors at one places which may have been covered up.
Barrow hospital had a serious problem with their maternity unit IIRC.
