Playing devil's advocate, Larsson had fouled Ramires so the ref was looking at where the ball had gone to see if he should play advantage?? If he sees Larsson go down out of the corner of his eye he can't know he was elbowed. His view could also have been blocked by Larsson's body. But typical that it happens against us and nothing happens.

There's a fourth official who must of been watching it and is still in contact with the ref.
